    I have carefully read, and understand, this Authorization of Background Investigation form and acknowledge receipt of the separate document entitled "A Summary of Your Rights under the Fair Credit Reporting Act" (a copy provided by Spike Enterprise or at https://www.comsumer.ftc.gov/articles/pdf-0096-fair-credit-reporting-act.pdf) and the "Applicant FCRA Disclosure Statement" and certify that I have read and understand both documents. By my signature below, I consent to the release of background reports and/or investigative background reports prepared by a consumer reporting agency, to Spike Enterprise and its designated representatives and agents for the purposes of determining my eligibility for employment, retention, or other lawful employment purposes. I further consent to the release of such background reports and/or investigative background reports to essential partners of Spike Enterprise, including but not limited to: industrial safety councils, and owners and/or operators of industrial facilities for which I may be asked to work on the premises of such industrial facility.     PRE-HIRE POLICY
    It is the policy of Spike Enterprise that after making a conditional offer of employment, but prior to an offer of employment the corporate office will check and verify the following information is correct and all necessary tests have been passed:

    Any material misrepresentation or omission on any employment application materials, including but not limited to the job application, or resume may be grounds for rejection of the application or termination of any subsequent employment with Spike Enterprise.
    NOTICE TO ALL NEW SPIKE ENTERPRISE EMPLOYEES
    It is Spike Enterprise's policy to recoup the cost of the pre-hire examinations and post hire training if your employment ends with Spike Enterprise prior to the completion of the 60 days probationary period with the Company. These costs can range from $125.00 to $735.00. All of these costs will be taken out of your final check. Your acceptance below indicates you are aware and have agreed to the following policy.
    NEW HIRE SUMMARY
    As part of Employee Orientation, engages new hires in the following:

    - We examine the general Safety Practices and Procedures.

    - The employee is given a company manual to read. We examine the manual with the employee to make sure that they understand it and answer any questions or concerns they may have, and then sign it. A current copy is available at employees request onsite.

    - The employee is informed of our random drug-testing and alcohol policies and asked to sign forms agreeing to them.

    - We advise them of our expectations while in possession of a company vehicle.

    - We explain the disciplinary program, dress code, and facial hair policies.

    - We stress the importance of a safe working environment as well as teamwork.

    - We inform the employee of the responsibilities of the individual employee.

    - We review the company policies and procedures.

    - We clarify the terms of our health insurance and benefits.

    - We advise them of the company's equipment and its safety procedures.

    - The supervisor discusses the working procedures of their responsibilities.

    Visual Safety:
    Trainees will be identified by a sticker or a different color hard hat or the procedures specific to the host facility for a period of 60 days or until the supervisor has deemed the trainee competent of fulfilling all the duties that the job entails. The trainee will also have a mentor to guide them through the working procedures of their responsibilities and answer and questions or concerns they might have. This identification will help all Spike Enterprise employees to guide the trainee through the safest day-to-day work procedures. This policy is seen to be a safety benefit not only to Spike Enterprise, but also to the companies that we are contracted to.

    Safety is Our Number One Concern:
    Here at Spike Enterprise, Safety is our number one concern. If anyone, from the trainee to the current employees, is seen using unsafe working procedures, it is everyone's responsibility to educate the employee as well as report it to their supervisor and/or the Corporate Office.

    I understand and agree that:

    1. The information that I have provided on this application is true and complete to the best of my knowledge. Any misrepresentation or omission of any fact in my application, resume, or any other materials, or during any interviews, can be justification of refusal of employment, or if employed, termination from Spike Enterprise's employ.

    3. Any offer of employment I may receive from Spike Enterprise is contingent upon my successful completion of the company's screening process, including, but not limited to my completion and passing of the following:

    - Receiving the Conditional Offer to Hire
    - Required background checks;
    - Pre-employment drug and alcohol testing;
    - Signing any and all documentation required for Spike Enterprise to complete its hiring process.

    4. I understand that as a condition of employment, I may be required to undergo and successfully pass a non-DOT screening for alcohol and/or drugs. I also understand and agree that, if employed, I may be required to submit to a non-DOT alcohol or drug screening at any time in accordance with Spike Enterprise's non-DOT drug and alcohol testing policies. I hereby consent to having the results of any such non-DOT alcohol or drug screening I may be required to undergo disclosed to Spike Enterprise.     6. In consideration of my employment, I agree to comply with the policies, rules, regulations, and procedures of the company, as may be amended from time to time. I understand that my employment and compensation can be terminated with or without cause or advanced notice, at any time, at the option of either the company or myself. I further understand that no manager or representative of the company, other than the President and CEO, has any authority to enter into any agreement with me for employment for any specified period of time or to make any agreement with me for employment for any specified period of time or to make any agreement different from or contrary to the foregoing.
