I authorize any person, school, current employer (except as expressly noted), past employer(s), and organizations named in this application form (and accompanying resume or other documentation, if any) to provide the Company with relevant information and opinion, personal or otherwise, that may be useful in making a hiring decision. I release all parties from all liability for any damage that may result from furnishing information and opinion to you.

I understand that as a condition of employment I may be required to take an alcohol / drug test if management reasonably suspects a condition exists that will prevent me from performing my job in a manner that does not endanger my own health or the safety and health of others.

I hereby authorize and consent to Sortpack Inc procurement of such report(s). I Understand that, pursuant to the Fair Credit Reporting Act, Sortpack Inc will, upon my request, provide me with a copy of any such report(s) if the information contained in such report(s) is, in any way, to be used in making a decision regarding my fitness for employment with Sortpack Inc. I further understand that, upon my request, that such report(s) will be made available to me , along with the name and address of the reporting agency that produced the report.

If hired, I agree to abide by all of the company rules and regulations, and I understand that, if employed, my employment may be terminated with or without cause, and with or without notice, at any time, at the option of either the company or me. I further understand that no representation, whether oral or written by any representative or agent of the Company, at any time, can constitute a contract of employment. I understand that the Company and all Plan Administrators shall have the maximum discretion permitted by law to administer, interpret, modify, discontinue, enhance or otherwise change all policies, procedures, benefits or other terms or conditions of employment. No representative or agent of the company, has the authority to enter into any agreement for employment for any specified period of time or to make any change in any policy, procedure, benefit or other term or condition of employment other than in a document signed by the President to make any agreement contrary to the foregoing.
