I hereby authorize and consent to Sol Shipping Services Inc procurement of such report(s). I Understand that, pursuant to the Fair Credit Reporting Act, Sol Shipping Services Inc will, upon my request, provide me with a copy of any such report(s) if the information contained in such report(s) is, in any way, to be used in making a decision regarding my fitness for employment with CHA Logistics Inc. I further understand that, upon my request, that such report(s) will be made available to me , along with the name and address of the reporting agency that produced the report
