The fact that you have a record of conviction will not necessarily bar you from employment.
Include the type of offense, date, place, arresting agency (Federal/State/City/County) and the outcome. You do not need to list convictions that a court has annulled.

I understand and acknowledge that any employment relationship with Cosmo Corporation is "at will", which means that an employee may resign at any time and that Cosmo can discharge an employee at any time with or without cause. I certify that the information contained in this application is correct to the best of my knowledge, and I understand that any false or misleading statement or omission of information is grounds for ending the hiring process or, if I am hired, termination of employment. I authorize verification of information provided on this application and release and hold harmless all parties from liability for any claims and, or damage that may result from furnishing same to Cosmo Corporation. I agree that any claim or lawsuit relating to my employment with Cosmo Corporation must be filed no more than six (6) months after the date of the employment action that is the subject of the claim or lawsuit. I waive any statute of limitations to the contrary.
