I voluntarily waive any right to receive written notice from any current or former employers prior to their disclosure of any disciplinary reports, letters of reprimand, or disciplinary actions relative to my employment within the last five (5) years. I understand that any offer of employment may be contingent upon my successfully passing a drug and alcohol-screening test at the request of Barracuda Staffing & Consulting. If I am offered employment, I agree to give my consent to a drug and alcohol screening test as required. A refusal to be tested shall be considered a voluntary withdrawal of my application for employment. Notice of Third Party Release of Information:
Barracuda Staffing & Consulting may be asked to release your phone number and/or email to a contracted client of Barracuda Staffing & Consulting for the purpose of a phone or virtual interview or as part of the client's interview process including administering testing or assessments. Additionally, the results from your pre-employment screening, to include but not limited to, pre-employment Drug Screening, Physical Screening, National Criminal Database Search, and Motor Vehicle Report results may also be released to a contracted Client of Barracuda Staffing & Consulting to whom you are being offered employment with as a part of the process of considering your candidacy as an employee. In the event that the information obtained in the reports are utilized in whole or in part in making an adverse decision with regard to your potential employment, we will provide you with a copy of the results obtained if requested.
